Authorization and Declaration

I/We hereby authorize PCCW Directories Limited (PDL) and its subsidiaries and affiliated companies to use all materials include but not limit to my name, photographs, product shots, logos, text, graphics and customer list I/we provided/shall provide for DIY amendment of this advertisement and to declare that:

1. I/We are the copyright owner of the items being uploaded and displayed in the advertisement or materials provided to PDL for the production of the advertisement AND/OR
2. I/We have been authorized by the copyright owner to display the trade names, photographs, product shots, logos, text, graphics, customer list or whatsoever in the advertisement.

I/We understand that the PDL and its subsidiaries and affiliated companies will not accept liability for any claim in the unlikely event of copyright infringement occurring directly or indirectly as a result of the materials provided for the DIY amendment of advertisement. I /We also agree to indemnify PDL and its subsidiaries and affiliated companies against any claim made against it that arises directly or indirectly as a result of any copyright infringement on the materials provided for the DIY amendment of advertisement.

I/We confirm that I/we am/are the owner of the company, or am/are authorized by the company to agree on the above on his/her behalf.
